Frank Zappa’s Welcome to the United States is possibly the only song about a form – the visa-waiver form you fill in when you enter the USA (do you still? It’s been a while). It asked you whether you are a Nazi or a communist, and whether you intended to overthrow the government.
I remember visiting Fiji in the early 80s, and their equivalent form asked you to declare if you were an anthropologist or a missionary (both sources of nuisance for a Pacific Islander I imagine).
